Survey for Students and Employees – Enter to Win Parking Permit or Equivalent Prize

A psychology faculty member is conducting an online study to learn more about the CSUSM community’s perceptions of smoking and other tobacco-related activities. The results of this survey will help inform the effectiveness of campus programs.

In appreciation for your participation, students and employees who complete the survey by Friday, Oct. 28 will be entered into an opportunity drawing, and one winner will have their choice of a semester parking permit, an iPad mini or Beats by DreTM headphones.

Take the Survey: http://www.surveygizmo.com/s3/2932222/Fall-2016-Campus-Tobacco-Policy-Survey

The survey will take approximately five minutes to complete, and will ask about your background, your perception of campus tobacco policies and your experience with tobacco use and secondhand smoke/vapor exposure. Participation is completely voluntary and you may discontinue your participation at any time. All responses will be kept strictly confidential, and results will only be reported in aggregate.

Thank you in advance for taking the time to complete this important survey.
